This booklet is a concise patient engagement tool that was developed to support implementation of the third WHO Global Patient Safety Challenge: Medication Without Harm. It is intended to engage patients in their own care in a more active way, to encourage their curiosity about the medications they are taking, and to empower them to communicate openly with their health professionals.
The tool focuses on 5 key moments where action by a patient or caregiver can reduce the risk of harm associated with the use of medications. These five key moments include: (i) starting a medication; (ii) taking my medication; (iii) adding a medication; (iv) reviewing my medication; (v) stopping my medication. It should be used in collaboration with health professionals, but should always remain with the patients, their families or caregivers. There are also implications for policy-makers in terms of systematic roll out of this tool.
