A simple tool to enable data collection on patient safety incidents using the Minimum Information Model for Patient Safety (MIM PS) approach.

This user guide has been developed to provide a simple tool to start collecting data on patient safety incidents to assist in data analysis and extract the minimal, but necessary, information to learn from incidents in order to avoid recurrence of same types of incidents in the future.

In addition, the Minimum Information Model for Patient Safety (MIM PS) can be used as a mapping source from any type of existing reporting systems that capture patient safety incidents, avoiding the need to develop new reporting systems based on MIM PS.
